Lagna (Ascendant) — where the chart begins
The rising sign
The lagna is the rāśi rising on the eastern horizon at the birth moment, and the first bhāva of the chart is counted from it. Phaladīpikā lists its names — lagna, horā, kalya, deha, udaya, rūpa, śīrṣa, vartamāna, janma — and each name is a statement of what it governs: the body, the appearance, the present, the birth itself.
Because a sign rises in roughly two hours, and a bhāva boundary can pass in minutes, the lagna is the one element of the chart that depends on the exact minute of birth. SIA computes it from the sidereal time of the birth place; the rāśi pages give Phaladīpikā's results for each sign as lagna.

लग्नं होरा कल्यदेहोदयाख्यं रूपं शीर्षं वर्तमानं च जन्म । वित्तं विद्या स्वान्नपानानि भुक्तिं दक्षाक्ष्यास्यं पत्रिका वाक्कुटुम्बम् ॥
TranslationLagna, Hora, Kalya (कल्य), Deha (देह) Udaya (उदय), Rupa (रूप), Seersha (शीर्ष), Vartamana (वर्तमान - living) and Janma (जन्म) are the names of the Ascendant or the first house. Vitta (वित्त- wealth), Vidya (विद्या - learning), Swa (स्व), Annapana (अन्नपान-riches food and drink), Bhukti (भुक्ति - (eating), the right eye (दक्षाक्षि - Dakshakshi), face (आस्य - Asya), letter or document (पत्रिका - Patrika), speech (वाक्-Vak) and Kutumba (कुटुम्ब) are appellations for denoting the second house.
The other lagnas
Parāśara derives further lagnas beside the udaya-lagna: the bhāva-lagna, advancing one sign every five ghaṭīs from sunrise; the horā-lagna, one sign every two and a half ghaṭīs; the ghaṭī-lagna, one sign per ghaṭī — each counted from the Sun's sign or the birth lagna by the rule the text gives — and the varṇada-lagna combined from the birth and horā lagnas. SIA computes these as part of the full gaṇita.

RenderingCount from sunrise, one sign for every five ghaṭīs, up to the birth time: if the birth lagna is an odd sign count from the Sun's sign, if even from the birth lagna — the sign reached is the bhāva-lagna.
घटीलग्नं प्रवक्ष्यामि शृणु त्वं द्विजसत्तम। सूर्योदयात्समारभ्य जन्मकालावधि क्रमात्।।२२।।

Questions
How exact must the birth time be?
To the minute, ideally. A sign rises in about two hours on average, so an error of a few minutes rarely changes the lagna itself — but it can move the lagna degree across a bhāva boundary and shift the divisional charts, which divide the sign finely.
What if I do not know my birth time?
SIA casts the chart for the time you give and marks the uncertainty; Lagna Shodhan then narrows the lagna from life events and traits, and states plainly when the evidence is insufficient.
